این پرسشنامه شامل 30 گویه می باشد دارای شیوه نمره گذاری به صورت کلید پاسخنامه همچنین تحلیل و تفسیر می باشد.
کلمۀ تخیل دقیقاً به توانایی ما در تشکیل تصاویر ذهنی اشاره دارد. در واقع به علت خاصی که تعداد معدودی از روانشناسان قادر به درک آن هستند ، هرکسی، توانایی تشکیل تصاویر ذهنی را ندارد. سه نفر از ده نفری که این کلمات را می خوانند ، شاید حتی مطمئن نباشند منظور از «تصاویرذهنی » چیست .
خیلی مختصر خود را بیازمایید : چشمهایتان را ببندید و سعی کنید قایقی را تجسم کنید که در پهنۀ دریای آبی ، بادبانی قرمز رنگ دارد. هفت نفر از ده نفری که این مطلب را می خوانند، از عهدۀ این کار بر می آیند اما سه نفر باقی مانده صرفاً نمی دانند چگونه چنین کنند.
اما تجسم تصویری تنها بخشی از تخیل است. تخیل شامل استعداد پیش بینی وقایع قبل از وقوع آن ، توانایی همدردی با دیگران در مواقع تلخ یا شیرین و البته ریشۀ اکثر فعالیت های هنری است.
